How often do Rahu and Ketu change signs?

About every 18 months. Because the nodes move retrograde, the axis shifts backward through the zodiac, so the houses they activate from your Moon change roughly once every year and a half.

Why are Rahu and Ketu always in opposite houses?

They are the two ends of a single axis — the points where the Moon's orbit crosses the ecliptic — so they always sit exactly 180 degrees apart. Whatever house Rahu transits, Ketu transits the seventh from it, which is why the two are always read together.
